Know a family who could use SDP help? For therapists, teachers, doctors, family resource centers and anyone outside the regional center who works with a family considering California’s Self-Determination Program. Here is what I do, what it costs the family, and the easiest way to connect us.

What I do

  • Person-Centered Planning built around the person’s strengths, goals and the life they want
  • Getting families from orientation to an SDP start date: paperwork, requests, budget and spending plan
  • Setting up the Financial Management Service, staff and vendors
  • Optional ongoing support after SDP starts

Who I work with

  • Self-advocates and families at all 21 California regional centers
  • Families just starting, and families already in SDP whose plan is not working

What it costs the family

Person-Centered PlanFunded by the regional center (service code 024)
Transition to SDPFunded by the regional center (service code 099)
Ongoing supportOptional, funded from the SDP budget

No out-of-pocket cost to the family. See the 099 rate at each regional center.

How to refer

The easiest way: the family reaches out

Hand the family the referral sheet or show them the QR code. It takes them to a short form where they can send me their own details, or they can call or text me directly at (714) 824-0453.

Questions about referring?

Call or text me about how referrals work. Please do not share anything about a family until they have agreed.
